| Initial Comments: The following deficiencies were observed at the time of the initial compliance inspection conducted on 12/1/2025, and are subject to changes pending programmatic review. The paper copy of the Notice of Inspection Rights was provided to the Licensee at the start of the inspection. The Licensee completed the Emergency Disaster Contact form. The following was discussed, but not limited to: *Nap mat/cot spacing must be maintained, *Feeding bottles must be labeled with a child's first and last name, *Diaper changing surfaces must be maintained free from items, *A diaper changing surface must be "next to" the diaper changing sink. Licensure is pending documentation of corrections, including photos. The plan of corrections is due within 10 days of receipt of this Statement of Deficiencies. | |||
