|Findings: Upon review of the children's emergency cards, Surveyor #1 observed that 3 of 10 were deficient: a) Children #1 and #2 were missing the name of of the individual to be contacted in case of injury or sudden illness of the child; b) Child #2 was missing the information on the back of the card regarding the nutritional and dietary needs of the child, the child's susceptibility to illness, physical conditions of which a staff member should be aware, and any individual requirements for health maintenance; and c) Child #3 was missing a date of enrollment.|

|Findings: Upon review of staff files, Surveyor #2 observed that Staff #2 (who started 8/31/15) had a new staff orientation form in her file, but it was completely blank.|
|Findings: The Surveyors observed that the times on the posted activity schedule did not match the schedule in the parent handbook, or the schedule that the staff reported. (All three were different.)|
|Findings: Upon review of the activity schedule and in speaking with the preschool staff, the Surveyors learned that the full-day preschool class did not eat lunch until 1:10 PM every day.|
|Findings: The Surveyors observed that there was a breakfast and lunch menu posted, but no snack menu.|
|Findings: Surveyor #1 observed that the first aid kit was missing sterile gauze pads.|
