During the inspection the principal provided the Licensing Specialist with a written statement explaining that the slide will only be used outside.

Findings: Upon review of child records, it is noted that neither female child, EB, or male child, EB, (siblings) have the required amount of immunizations for their respective ages. Female EB is missing four (4) vaccinations while male EB is missing eight (8) vaccinations respective to their age. The male child, EB, has not had at least one dose of DTAP, Polio, Hib or Prevnar. According to G(2) of this regulation, he should have had at least one dose of each of these before being admitted to the Center for care. At the time of the inspection, the Center could not provide any evidence that he is scheduled for any immunizations. Please have the parent rectify the lack of proper immunizations for both children as soon as possible.
