Noncompliance Area: In the Preschool/School age room, wooden blocks were observed to have splinters and rough edges. In the Infant/Toddler room, a plastic toy bin containing foam blocks was observed to have a cracked handle, exposing the children to rough edges and potential pinch points. In the Gross Motor Space, a car table was observed to have cracks in the wooden frame, exposing children to splinters and potential pinch points.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The files for Child #2 and Child #4 were observed to contain original, signed agreements between the parent and the operator.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement. A child's record shall contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement.

Noncompliance Area: Children were observed in the Gross Motor Space without emergency contact forms being present; it was reported that the emergency contact forms had been left in the children's main room.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: The health assessment in the file for Child #3 lacked documentation of an up-to-date flu vaccination in accordance with the scheduled recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Mylicon Gas Relief Drops for Child #1 were observed in the medication box in the Infant/Toddler Room without a medication log.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: The interconnected fire alarm system in the facility was not operable in the Gross Motor Space. A battery operated smoke detector was observed in the space, however lacked batteries and was not operable.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: An expired certificate of compliance was observed in the facility at the location used by parents for pick up and drop off.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.

Noncompliance Area: The emergency plan lacked updated information and evacuation routes for the added Gross Motor Space.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility.

Noncompliance Area: The file for Facility Person #2, start date 10/14/2019, contained a Department of Education FBI Clearance and lacked a Department of Human Services FBI Clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #2 may not work in a child care position at the facility.

Noncompliance Area: The file for Facility Person #1 contained a most recent staff evaluation dated 10/17/2018; lacking a staff evaluation within the last 12 months.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: Trash receptacles in the Preschool/School Age Room and the Infant/Toddler Room that contained trash that had been contaminated by human secretions were observed without lids.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Noncompliance Area: The telephone in the Preschool/School-Age Space was observed to not have emergency telephone numbers posted at the location of the telephone.

Correction Required: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility.

Noncompliance Area: The first aid kit in the Preschool/School Age Room and both first aid kits in the Infant/Toddler room lacked tweezers.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The second bathroom in the Gross Motor Space did not have hand washing signs posted at the toilet or the sink.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: The evacuation routes posted throughout the facility did not include the Gross Motor Space and the exit located within the Gross Motor Space.

Correction Required: Evacuation routes shall be posted.
