Noncompliance Area: During the hybrid renewal inspection conducted on 10/1/20, the information received and reviewed from the file of staff member #2, did not include a current health assessment. The health assessment reviewed showed a date of last exam on 4/12/18.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: During the hybrid renewal inspection conducted on 10/1/20, the facility did not provide a current copy of the general liability insurance. The insurance document provided expired on 4/24/20.

Correction Required: The legal entity shall have comprehensive general liability insurance to cover the persons who are on the premises. A current copy of the insurance policy shall be on file at the facility.

Noncompliance Area: During the hybrid renewal inspection conducted on 10/1/20, the information received and reviewed from the file of staff member #1 did not include an updated and current copy of the Mandated and Permissive Reporting in Pennsylvania training. The training provided from the file of staff member #1 for review of the Mandated and Permissive Reporting in Pennsylvania was last completed on 7/16/15.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: During the hybrid renewal inspection conducted on 10/1/20, based on the staff information provided for review, the facility did not provide documentation of staff having current first-aid qualifications. Adult/Pediatric First-Aid CPR training reviewed dated for 2/15/18 and valid for 2 years only.

Correction Required: One or more facility persons competent in first-aid techniques shall be at the facility when one or more children are in care.

Noncompliance Area: During the hybrid renewal inspection conducted on 10/1/20, the hot water temperature measured at the sink of the bathroom used for children measured 115.3 F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: During the hybrid renewal inspection conducted on 10/1/20, a water damaged ceiling tile was observed located in-between the indoor play space/kitchen area.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: One cracked lid with a potential pinch point was observed in the front classroom.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: A Bumbo seat that was recalled due to the need for a safety strap was observed. The required safety strap was not in place.

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement.

Noncompliance Area: The files for the two children enrolled included the original agreements.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement.

Noncompliance Area: A hands free covered can was not in place at the chaning table in the orange restroom.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: A written feeding schedule for an enrolled infant was not on file at the time of the inspection.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: The emergency contact form on file for child #1 did not include signed parental consent for administration of minor first-aid procedures by facility staff.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: A tube of desitin diaper cream was observed in a cubby in the front child care space.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The hot water in the restrooms was measured at 114.5 F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: Small areas of peeling paint were observed on the blue wall in the front child care space. Damaged plaster was observed on the bottom right corner at the gate from the gross motor to the back child care space.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
