Findings: At the time of the inspection, 2 of the 5 children enrolled did not have their immunization records, their Health Inventory Part I or II, or their lead screening record. Provider stated that they had just started yesterday. Another child who had been in care for a year did not have his Health Inventory Part II in his file. Provider sent a FAX on 12/20/2021 with required medical paperwork for the 2 children that were missing HI parts I and II, the immunizations and lead screening forms completed by the doctor. On 1.4.2022, LS received an email from provider stating that parent of child that was lacking HI II was disenrolling the child. Final noncompliance was marked as corrected. |
