Noncompliance Area: Child 1 emergency contact form was missing the work address and telephone number.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Noncompliance Area: During my walk through I observed that there were no names on the children's cups.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Child 1 file was missing consent for special diet and medication .

Correction Required: A child's record must contain signed parental consent for administration of medications or special dietary needs.

Noncompliance Area: While reviewing child 1 file I observed that consent for swimming and wading was not giving, or declined.

Correction Required: A child's record must contain signed parental consent for transportation, walking excursions, swimming and wading.

Noncompliance Area: Provider informed me that she had not sent a copy of her plan to the emergency management agency.

Correction Required: The operator shall send a copy of the emergency plan and subsequent plan updates to the county emergency management agency.

Noncompliance Area: During my walk through I observed several electrical outlet with out covers in the vestibule.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: While reviewing the first aid kit I observed that it was missing soap.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: At the time of the renewal inspection, the refrigerator thermometer was missing.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: At the time of the renewal inspection, it was observed that one child was in the living room watching television while the other three were in the front room with the provider playing and the provider could not adequately supervise the child.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3290.51-3290.52 (relating to maximum number of children; and staff:child ratio). Children must be supervised at all times.

Noncompliance Area: At the time of the renewal inspection, the agreement forms for children #1 and 2 were not in the children's files.

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: At the time of the renewal inspection, the agreement forms for children #3 and 4 did not include the date of the children's admission.

Correction Required: An agreement shall specify the date of the child's admission.

Noncompliance Area: At the time of the renewal inspection, the emergency contact forms for children #3 and 4 did not include any information on the child's special needs, if any.

Correction Required: Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant of CRNP, which is needed in an emergency situation.

Noncompliance Area: At the time of the renewal inspection, the file of staff #1 had a health assessment that was dated 9/8/16 which is more than 24 months old.

Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: At the time of the renewal inspection, a copy of the regulations and how to contact the regional office was not posted in a conspicuous location used by parents.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: At the time of the renewal inspection, the emergency plan did not provide a method for provider to contact parents in case of an emergency or a method to contact parents when the emergency is over.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Noncompliance Area: At the time of the renewal inspection, the emergency plan had not been reviewed and documented annually

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: At the time of the renewal inspection, the file of staff #1 had only 3 hours of child care training in the past year. 3 additional hours are needed to meet the annual requirement of 6 hours.

Correction Required: A staff person shall obtain an biennial minimum of 12 clock hours of child care training.A facility person's record shall include records of training required by the Department.

Noncompliance Area: At the time of the renewal inspection, cleaning and other toxic materials were observed in the bottom of the bathroom vanity and on the counter where it was accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: During renewal inspection certification representative found that household pet, dog, did not have a file with vaccination report present at the facility

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: In review of the children's files it was found that child #7 did not have an agreement form on file

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: In review of the children's files it was found that child #2, #3, #5, #6 and #7 did not have documentation of child service report on file

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: During renewal inspection certification representative observed a total of 7 unrelated children in care, 2 infants and 5 toddlers ( 3 young toddlers and 2 older toddlers)

Correction Required: The number of children in care may not exceed six children at any one time who are unrelated to the operator.The operator may provide care to no more than five related and unrelated infants and toddlers at any one time. No more than two related and unrelated infants may receive care at any one time. If two infants are in care, three toddlers are permitted.

Noncompliance Area: At time of inspection the certification representative questioned provider about rest equipment, no age- appropriate rest equipment is available at the facility, blankets are used on the floor

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child`s parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.Bed linens may not be used alone as age-appropriate rest equipment.

Noncompliance Area: At time of inspection the certification representative observed the provider go into the kitchen while children are in livingroom area, all children cannot be observed at one time

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. TIERED LIS: Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format.

Noncompliance Area: At time of inspection veterinarian`s certificate of current rabies immunization, for two dogs, was not on file at the facility

Correction Required: A veterinarian`s certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: At time of inspection the certification representative observed an infant asleep on the couch, another infant asleep in a car seat.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: In review of the children's files it was found that child #3 did not have the boxes checked stating complete enrollment packet or updating every 6 months on the agreement form

Correction Required: The operator shall review with the parent, at the time of application, the facility`s general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.At the time of enrollment, a parent shall receive in writing the facility`s general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: During inspection certification representative observe that the emergency plan did not have documentation of shelter in place. The plan did not have written documentation of update and review

Correction Required: The facility shall have an emergency plan that provides for shelter of children during an emergency including shelter in place at the facility and shelter at locations away from the facility premises..The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: In review of the staff and household members files it was found that facility persons #1, #2, and #3 did not have documentation of FBI clearance on file. Facility person #3 did not have Police clearance, Child abuse clearance or FBI clearance on file

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person`s record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: During inspection certification representative observed that ratio requirments are not being kept, at time of inspection the operator had 3 infants, 1 YT, 1 OT and 1 newly turned 3 year old.
