Description: Your child should have brought the consent for, the Vaccine Information Sheets for the five vaccines that are required for 7th grade or recommended for your child’s age (these explain the benefits and risks of each vaccine), and information sheet highlighting the vaccines that are required and recommended for your child form home on Tuesday the 19th. Please contact the school nurse if you need these forms.
