Noncompliance Area: The emergency contact form in the file for Child #2 lacked signed consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: The file for Child #3 lacked a completed health assessment within 60 days following the first day of attendance at the facility.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The file for Child #1, an infant, contained health assessments dated 03/13/2019 and 12/12/2019; more than six months passed between health assessments.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: The file for Child #2 lacked documentation of up to date flu vaccinations in accordance with the schedule recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The emergency contact form in the file for Child #2 lacked signed consent for administration of minor first-aid procedures.

Correction Required: A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: The emergency contact form on file for child #4 was observed not to have documented the phone number of the child's physician or source of medical care.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: The file for child #3 an older aged toddler was observed not to contain documentation of a health report being completed within the last 6 months. This is evidenced by the most recently dated health report on file for child #3 being dated 2/20/2018. This is a period greater than 6 months.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: The health report on file for children #1 and #2 were observed not to include a statement stating that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: The immunization record on file for children #1, #4 and #5 were observed not to be up to date with the recommendations of the ACIP this is evidenced by the following; the immunization record on file for child #1 was observed not to contain documentation of child #1 receiving an influenza vaccine within the last 12 months nor was there documentation on file of child #1 receiving a second dose of the Hepatitis A vaccination by age 2 as recommended by the ACIP, the immunization record on file for child #4 was observed not to contain documentation of child #4 being vaccinated with the influenza vaccine within the last 12 months nor was there documentation of child #4 receiving a second dose of the Hepatitis A vaccination by the age of 2 as recommended by the ACIP, also the immunization record on file for child #5 was observed not to contain documentation of child #5 receiving the influenza vaccine within the last 12 months as recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: In the play room downstairs a toy bin containing foam puzzle pieces was observed to be cracked along the edges creating rough edges, sharp corners and pinch an crush points.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: It was observed that the infant cribs located in the infant room were not labeled for the children whom they are intended for.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: It was disclosed by facility staff that there is not a written statement on file at the child care facility giving the formula a feeding schedule for each infant aged child in care.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: The left wall at the entrance of the reading nook in the preschool room was observed to have damaged plaster and peeling paint.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: **At approximately 9:00am it was observed that there was one staff person in the downstairs room to the right of the staircase with 4 preschool children. The staff person was in the rear of the room. It was also observed that there was one preschoolers in the bathroom off of the downstairs room to the left of the staircase who was not being supervised by a staff person. The staff person present in the downstairs room to the right of the staircase could not see, hear, direct or assess the child who was in the bathroom off the downstairs room to the left of the staircase.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training.

Noncompliance Area: At approximately 10:20am it was observed that Staff person #1 was alone in the nursery room with 4 young toddlers and 1 infant. Staff person #2 admitted that at that same time, she was alone in the downstairs room with 2 older toddlers and 7 preschoolers.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).
