Noncompliance Area: At approximately 2:45 pm staff person's #1 and #2 were observed caring for a group of 9 children consisting of 6 infant aged children and 3 younger toddler aged children in the infant room.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.

Noncompliance Area: 2nd Citation - Previously cited on 3/20/2019. On 6/28/2019, in the outdoor play space peeling paint was observed on the door jamb located at the far right end of the outdoor patio when looking at the building.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: On 3/20/2019, the agreement form on file for child #2 was observed not to contain documentation of the date on which the fee is to be paid.

Correction Required: An agreement shall specify the date on which the fee is to be paid.

Noncompliance Area: On 3/20/2019, the emergency contact form on file for children #1 and #3 were observed not to contain documentation of the enrolling parent's work address and phone number.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Noncompliance Area: On 3/20/2019, A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency were observed not to be posted in a conspicuous location in the infant/toddler classroom.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: On 3/20/2019, the health report on file for child #1 was observed not to contain a dated signature by the child's physician, physician assistant or CRNP.

Correction Required: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title.

Noncompliance Area: On 3/20/2019, the immunization record on file for children #2 and #4 were observed not to contain documentation of the children being up to date on their immunizations as recommended by the ACIP. This is evidenced by the immunization record on file for child #2 not showing documentation of child #2 receiving a 3rd dose of DTap and PCV13 as recommended nor documentation of child #2 being administered an influenza vaccine within the last 12 months, and the record on file for child #4 lacking documentation of administration of a 4th dose of DTap or administration of an influenza vaccination within the last 12 months.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: On 4/12/2019, the file for staff person #1 was observed not to contain documentation of a complete Pennsylvania State Police criminal history clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 1 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility.

Noncompliance Area: On 3/20/2019, on patio of the outdoor play space a loose hanging cable was observed to be hanging from the building creating a visible hazard to the children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: On 3/20/2019, on the patio of the outdoor play space peeling paint was observed on the door jamb of the door on the far right of the building, to the left of the same door on the brick, and above the two glass block windows in the middle of the building.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: The pack and plays located in the infant room were observed not to be labeled for the child whom they are intended for.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The pack and plays where infant aged children were napping in the infant room were observed to have loose fitting sheets placed on the resting mat, creating a suffocation hazard to the children napping in the equipment.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: There were foam blocks with teeth marks and small chunks missing from them in the infant/toddler room.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: There was no emergency transportation procedure posted in the infant/toddler room.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: There was diaper cream and children's pain relief medication accessible to children in a back pack on the floor in the preschool room.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.
