Violation: The facility's outdoor space was not safe for children's activities as evidenced by there is a new shed that has been installed in the outdoor fenced in playground. The shed presents a hazard for the shed is several inches off of the ground. Children have access underneath the shed on the front side of the shed (9 inches), the left and right side of the shed (8 inches) and towards the back of the shed where the shed is not flush up against the fence there is an 8 inch gap between the shed and the fence. The gaps underneath the shed and on the sides of the shed pose a risk for entrapment.

The Tuberculosis (TB) Risk Assessment form, revised March 2014, is incorporated by reference in this rule, as published by the Missouri Department of Health and Senior Services, PO Box 570, Jefferson City, MO 65102 and available by the Missouri Department of Health and Senior Services at https://health.mo.gov/living/healthcondiseases/communicable/tuberculosis/tbmanual/pdf/RiskAssessmentform.pdf. If the person has signs or symptoms of tuberculosis, or risk factors for tuberculosis, then testing for tuberculosis shall occur.
