Where possible, ChildcareCenter provides inspection reports as a service to families. This information is deemed reliable, but is not guaranteed. We encourage families to contact the daycare provider directly with any questions or concerns, as the provider may have already addressed some or all issues. Reports can also be verified with your local daycare licensing office.
|Findings: All children enrolled needed documentation in their files that their parents have received the consumer pamphlet. The director will give parents an addendum to their contract with the information on how to file a complaint if they needed to. The licensing specialist will be notified when this is completed.|
|Findings: Four children need Part1 of the Health Inventory form completed by the parents. Three children need Part 2 of the Health Inventory form completed by the physician. Twenty-nine children need page 4 of the Health Inventory form completed by the parent and the physician if required. When these items are completed the licensing specialist will be notified.|
|Findings: Two children need immunization records. Five children need updated immunization records, they are all missing an MMR and a Varicella vaccine. When these get completed, the director will notify the licensing specialist.|
|Findings: The school age room and the small preschool rooms need updated staffing patterns posted. The larger preschool room needs a staffing pattern posted. There was none in that room. The director will create updated staffing patterns, post them in the classrooms and send copies to the licensing specialist.|
|Findings: There were four staff members who are no longer at the center and there was no notification of this to the licensing specialist. One staff left in January. Another staff member left in May and two others left in August. The director did complete a staff personnel change form during this visit, deleting these staff members and gave it to the licensing specialist.|
|Findings: The baseboard radiators ,which are not functional, need to have coverings placed over them since the ones in the large preschool room are having the front metal coverings falling off, exposing sharp surfaces. The director stated that she was planning to cover them. When this is completed, pictures will be sent to the licensing specialist.|
|Findings: The exhaust vent in the school age bathroom needs to be cleaned. When this is completed the licensing specialist will be notified and pictures will be sent showing that this was completed.|
|Findings: The stove and toaster oven in the kitchen had food on them. The licensing specialist requested that the director clean these so no infestation would occur. The director will do this and send pictures to the licensing specialist showing that these have been cleaned. A dirty pot was removed from the stove by the director and soaked in the sink during this visit.|
|Findings: One of the preschool teachers was late on their training for this year. It should have been completed by September 30, 2018 and she currently is finishing the on-line class. The other preschool teacher has until July 31, 2019 to complete the required number of hours. When the training is completed, copies of the training certificate will be sent to the licensing specialist.|
|Findings: The last documented fire drill was March 3, 2018. The director will make sure that all fire drills are recorded. She claims that they were done but not documented. She will update the schedule and document the fire drill for November. When this is completed, the licensing specialist will be notified. One emergency drill was documented in February. A reminder that another emergency drill was needed before the end of the year was given to the director.|
