Findings: The basement is currently a classroom for preschool/school age children. The stairs open directly to the basement classroom. There is no barrier or gate at the bottom of the stairs. At one point a child was having some distress and the licensing specialist heard him crying and opened the basement door and observed a child sitting on a step close to the top of stairs and a staff person trying to console him, they then went back down the stairs into the classroom. Please place a gate or barrier at the bottom of the stairs as soon as possible and send a picture to the licensing specialist once completed. | |||

Findings: A complaint was received alleging that a child fell out of a high chair because they were not strapped in the chair. The staff person that was in charge of the child at the time of the fall, is no longer employed with the center. This Licensing Specialist (LS) interviewed the other staff person that was in the classroom when the incident occurred. This staff person said that she was tending to another child and the staff person in charge of the child that fell had put the child in the high chair, stepped back about 2 or 3 feet to get the snack. She then heard a thud and asked the staff person if the child had fallen and she responded yes. The child cried but there were no visible injuries. The mother was called and arrived a couple of hours later. No medical attention was sought. After this incident the director advised the staff to feed the child from the ground level chair. The director reported to this LS that the staff person had put the child in the high chair, went to grab her phone to add information in Procare and that is when the child fell. It seems the staff person did not provide the appropriate supervision to the child or use the high chair restraints properly. | |||

The oldest child is supposed to transition to the toddler room in January. The pillows and blankets were removed during the inspection.
