Findings: The provider has two individuals residing in her home that the office was not properly notified of. One of the individuals is the provider's grandchild who was born in July 2018, which the provider failed to notify this office of this child's birth. Regarding the other individual is an adult resident. The office was notified that there was a new adult living in the home; however, no personal information was provided on who this adult was. The Licensing Specialist requested this information on numerous occasions. At today's inspection, an original notarized release of information was provided on this individual, which was submitted on an outdated form. Please ensure that current forms are being utilized. As discussed at today's visit, submit a medical evaluation on these two residents. | |||

Findings: The outdoor play set was observed by the Licensing Specialist and the provider at today's inspection. The play set has steps from the ground that lead to a 6 foot platform. The steps leading to the platform have a gap between each step ranging 6-7 inches, which pose concerns of entrapment. This concern was noted on the June 2017 Mandatory Review inspection. On March 22, 2018, the office received a letter of correction which noted that this concern was corrected on 8/21/2017. It was observed at today's inspection, that the provider had installed a 2 by 4 piece of wood mounted vertically behind the steps; however, the play set continues to have entrapment concerns. The Licensing Specialist showed the provider the continued entrapment concerns and how a child could potentially get trapped between the steps and to the sides of the board that was installed. The provider stated she will install 2 additional pieces of the 2 by 4 wood, vertically behind each step to prevent this entrapment concern. There will then be 3 pieces of wood mounted and secured vertically behind the steps. Please submit a photograph to the office in lieu of a follow-up inspection. |
