Noncompliance Area: There was peeling paint on the ramp railing along the walkway to the outdoor play area. There was also broken lattice assessable to underneath the ramp; during the inspection.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: At the time of inspection, facility children #1-#3 were observed unsupervised playing on the computer in the front (right) room. Staff #1 said they were using the restroom.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times.

Noncompliance Area: Broken toys with sharp edges were observed on the facility play space at the time of inspection that could pose a hazard to the children.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During the inspection, preschool children were observed sleeping on mats that were not labeled for individual use.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The health assessment in the staff #2 was incomplete at the time of inspection. All required questions were not answered .

Correction Required: An adult individual who is employed by a facility and who provides children with social, medical, psychological or psychiatric services in addition to this chapter is required to have a current health assessment on file at the facility. An adult individual or an employee of an agency who provides those services by contract with the child's parent or the facility is not required to have a current health assessment on file at the facility.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: The DHS-OCDEL certificate of compliance was not posted in a visible location at the time of inspection on 5/7/19.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.

Noncompliance Area: When checked at the time of inspection on 5/7/19, the menu posted of the facility refrigerator was dated for February.

Correction Required: The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: At the time of inspection, staff #1 did not have clear/legible proof of complying with the CPSL (FBI) requirements.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: During the inspection on 5/7/19, the file for staff #2; who was observed working in the classroom did not have valid proof of education.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing.

Noncompliance Area: The file for staff #3 (director) did not have adequate written proof in their file at the time of inspection on 5/7/19.

Correction Required: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week.

Noncompliance Area: At the time of inspection on 5/7/19. The fence in the facility play space in the rear of the building was broken. There was also missing lattice along the ramp that could pose a hazard to the children in care.

Correction Required: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.

Noncompliance Area: During the inspection, there were areas with broken plaster in the facility near the lunchroom. There were soiled pak-n-play's in the infant area.
