on 11/17/2023 a bottle was mistakenly fed to the wrong baby. We have two children in the infant 2 classroom with the same first name. Around 4:00pm our Assistant Director transferred an infant from Infant 1 to Infant 2 in order to maintain ratio. She asked if the child would be due for any bottle or diapers and was informed that the infant was due for a bottle around 4:10-4:15. A teacher in Infant 1 handed her another infants bottle. Both infant's bottles were taken to Infant 1. When the infant was due for a bottle, they were fed the other child's bottle of breastmilk. Both families were notified immediately. We self reported both to our state consultant and our school business consultant. We reviewed all proper bottle verification procedures with the staff involved. We reviewed with all staff one Monday 11/20/2023 at our staff meeting. We since made teacher changes in order to provide more consistency in the classroom and we will be working with a Spark consultant to disuss any other precautions we can take to prevent this from reoccuring.
