Plan: Caregiver will talk to the owner of the house about the entry ceiling, kitchen floor linoleum and ceiling fan. She will request that these items be repairs as soon as possible. Caregiver will continue using various methods to rid the home of roaches (as noted in discussion) and will sweep up dead roaches throughout the day as they are noticed. The floors will be swept or vacuumed throughout all areas used by children and free of all small debris. The home will be kept in a clean and sanitary condition and in good repair.

Linoleum in front of kitchen sink has a large area missing and bare concrete is exposed. The ceiling fan in the dining area is hanging loose from the ceiling by a few inches with wires exposed. The drywall on the ceiling in the front entry area is sagging. There are multiple live and dead roaches in the kitchen and dining area on the walls, floor and sink area. A few dead roaches are present on the floor in the entry area and a few live roaches were observed on the wall in the front main play room; there are signs of a roach infestation. The floors in the kitchen, main front play room, hallway and bathroom are not clean and sanitary and have small pieces of debris present throughout (in addition to the roaches in the kitchen and dining area).
