Noncompliance Area: There was a high chair in the infant room that lacked a t-shaped safety strap.

Correction Required: High chairs shall have a wide base and a T-shaped safety strap.

Noncompliance Area: The utility closet door in the preschool room was unlocked allowing paint cans to be accessible to the children.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: The facility's fire drill log documented a fire drill on 12-2-19 and not again until 2-18-20, which was more than 60 days apart.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: The refrigerators in the preschool room, older toddler room, and kitchen had shelves on which there were sticky substances, dirt and debris.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: There was a diaper cream in the infant room that was not labeled with the intended user's name.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: Staff person #3 had a previous health assessment dated 8-30-16 and a current health assessment dated 4-24-19.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Staff person #4 had no childcare training between 9-8-17 to 9-8-18.. Staff person #4 had 6 hours of childcare training in April of 2019 which will be used to meet the requirement between 9-8-17 and 9-8-18. Staff person #3 had no childcare training hours between 3-27-18 to 3-27-19. Staff person #3 had 6 hours of training in April 2019 which will be used to meet the requirement between March of 2018 and March of 2019. Staff person #2 had no childcare training hours between 5-1-18 and 5-1-19.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.

Noncompliance Area: Staff person #1, with a documented start date with children of 5-6-19, lacked a disclosure statement. Staff person #2, with a documented start date with children of 5-1-18, had a record that lacked a child abuse clearance and had a mandated reporter training that was dated 12-31-18.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: Staff person #2 lacked an annual written evaluation.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: It was disclosed by staff person #1 that the regional office of child development and early learning was not notified with 24 hours after staff person #1 was notified by the parent of child #1 that child #1 had received medical care at the emergency room from an injury that occurred while in care on 11/5/2018.

Correction Required: The operator shall immediately notify a child's parent and shall telephone notice to the appropriate regional office within 24 hours if a child in care at the facility is hospitalized or receives emergency room treatment.

Noncompliance Area: There were no emergency contact forms present in the front infant room for the children receiving care in that room.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: The written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency were not posted in the front infant room.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: There was peeling purple paint on a wooden decorative toy truck located outside of the main entrance of the facility.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: There was a cracked bin filled with toys in the preschool room and in the older toddler room.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: At 10:30am it was observed that Staff person #1 was alone in the infant room with 5 infants and 1 young toddler. It was reported that Staff person #2 left the infant room to go into the kitchen to prepare a child's bottle.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).
