Please complete the Plan of Corrections on the Licensing portal within 10 days of receiving this Statement of Deficiencies. The following items were discussed, but are not limited to: *Items required in a transportation vehicle *Licensee will submit a space change application for approved facility areas

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 5/30/2024 and are subject to changes pending programmatic review. Please submit the Plan of Correction via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *A blanket being used during tummy time *The temperature of shredded tire pieces on the outside playground *Ensuring the second page of the Criminal History Affidavit is present *Ensuring toothbrushes and diapers are not kept in the bathroom Primary Compliance Officer is AuReyon Thompson.
