Findings: Health and Safety Concerns. There were concerns that there were cases of lice and bed bugs at this facility. The health department performed an investigation and determined this was substantiated. Resolution: The health department determined that this provider reviewed health policies with staff and families. The facility was cleaned thoroughly to mitigate the bed bugs and children with lice were sent home until cleared of nits.

Findings: Supervision Concerns. There were concerns that during nap time, two young children were showing each other their privates and that staff did not intervene in a timely manner. The health department reviewed the footage and determined this incident had occurred and that the staff member was not actively supervising the children in order to intervene quickly. No other supervision or health & safety concerns were observed during the investigation. Resolution: The provider notified the parents and held an all staff meeting to review active supervision and reporting policies.
