Noncompliance Area: The most current rabies immunization record on file for Cass, the cat, expired on 7/6/2020.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: There was more than 6 months in between the previous and most current child service report on file for child #4 and was due in the file prior to regulatory suspension date of 3/6/2020. Child service report was completed 4/14/2019 and then on 6/29/2020.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: There was more than 12 months in between the previous and the most current and updated health assessment on file for child #3, who is of preschool age and was due in the file prior to regulatory suspension date of 3/6/2020.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record must contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement.

Noncompliance Area: The most current and updated health assessment on file for child #2 lacks a statement that the child is able to participate in child care and appears to be free from communicable disease.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: In the downstairs play room and by the play tire, observed a hanging cord in reach of the children. This poses a strangulation hazard to the children in care.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: There was more than six months between reviews of the emergency contact information and fee agreement on file for children #1, 3, and 4. Child #1 and #4's emergency contact information and fee agreement were reviewed on 4/16/2019 and then on 6/8/2020. Child #3's emergency contact information was reviewed on 4/18/2019 and then on 6/10/2020 and fee agreement was reviewed on 4/6/2019 and then on 6/10/2020.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The most current mandated reporter training on file for staff #1 was not completed within the previous 60 months. Most current mandated reporter training is dated 6/12/2015.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include records of training required by the Department.

Noncompliance Area: Observed two toy bins, one green and one purple, in the outside play area. There were multiple cracks on the toy bins causing a pinch point.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: There was more than 12 months in between the current and updated health assessment and the previous health assessment on file for child #2, who is of preschool age.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record must contain initial and subsequent health reports.

Noncompliance Area: The current immunization record on file for children #1, #2, and #3 lack verification of a flu vaccine within the past 12 months.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: A copy of the current applicable regulations under which the facility is certified was not posted.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: In the back yard where the children were playing, observed peeling paint on the bottom right side of the deck.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: In the bottom floor play area, observed that the door to the laundry room was unlocked. On the other side of the door and accessible to the children in care, observed Lysol Spray, Pledge Cleaner, Detergent Pods, and Windex which were all labeled as Keep out of Reach of Children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
