Noncompliance Area: The facility file for the cat named Mumma lacked verification of a current rabies immunization. New rabies immunization was due by 4/15/2019. The facility file for the dog named Harley lacked verification of a current rabies immunization. New rabies immunization was due by 4/11/2019.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: There was more than six months between reviews of the emergency contact information and fee agreement on file for child #1. Reviews were 1-16-2019 and then 1-27-2020.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The file for child #1, who is of preschool age, lacked an updated health assessment which was conducted in the last 12 months. Last health assessment was completed 2/26/2019.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record must contain initial and subsequent health reports.

Noncompliance Area: The most current immunization record on file for child #1 lacked verification of flu shot within the past year as in accordance with the schedule recommended by the ACIP. The most current immunization record on file for child #2 contained verification of 1 Rotavirus when 2 are required, 1 DTAP when 2 are required, 1 HIB when 2 are required, 1 Pneumococcal when 2 are required, 1 Polio when 2 are required, and verification of a flu shot within the past year as in accordance with the schedule recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: On the date of inspection, it was observed that there was not an operable smoke detector in the basement because the smoke detector was not hung on the wall as required by manufacturer instructions and was sitting on the furnace. This condition at the facility poses a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.. Demonstration of Compliance with 34 Pa. Code 403.23 is required. . The operator will verify correction with an agent of the Department after installing an operable smoke detector in the basement.

Noncompliance Area: The last review of the emergency plan was more than one year ago and dated 2/26/2019.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: The file for staff person #1 lacked 3 of the required biennial clock hours of child care training and were due in the file by 1/1/2020.

Correction Required: A staff person shall obtain an biennial minimum of 12 clock hours of child care training.A facility person's record shall include records of training required by the Department.

Noncompliance Area: Observed that the wooden front steps, wooden porch floor, and wooden porch railings had peeling paint. These areas are used by the children in care to access the family child care home.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Observed a child's chair which had foam on the seat and was covered with plastic. Half of the plastic was coming off of the chair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the back yard, observed an embedded swing set which lacked loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Observed an infant pack n play which was not labeled with the child's name.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The facility refrigerator lacked a thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Based on a conversation with the operator and observation, child #1 who is an infant, sleeps in a bouncy chair.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: The facility lacked a conspicuously written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of emergency.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: Child #1, an infant, lacked a written statement from the parent giving the formula and feeding schedule.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: Observed an infant's bottle which was not labeled with the child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: In the small child care room, observed small dime size areas of damaged plaster above the toy box.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: In the living room, observed a pack-n-play obstructing the front exit door.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.

Noncompliance Area: The facility file lacked a veterinarian's certificate of current rabies immunization for a cat at the facility named Mamma.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: The file for child #1, who has been enrolled longer than six months, lacked a child service report within the past six months.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: Observed downstairs bathroom used by the children in care. Based on a conversation with the operator, the children use paper towels to dry their hands. However, there were no paper towels present in the bathroom.

Correction Required: Paper towels may be used as towels and washcloths. Paper towels shall be discarded after each use.

Noncompliance Area: The facility's current certificate of registration was not posted in a conspicuous location used by parents.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: The last review/update of the emergency plan was 05-01-2016.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: The first-aid kit lacked tape.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: In the small playroom, observed a portable space heater. Based on a conversation with the operator, the space heater was used with children in care the day prior to inspection. There was no verification that space heater was allowed by local ordinance and the manufacturer's instructions for use were not kept in an accessible area.

Correction Required: Portable space heaters, if allowed by local ordinance, may be used while children are in care, if the units are used in accordance with the manufacturer's operating instructions.The manufacturer's instructions for use shall be kept in an accessible area in the facility.

Noncompliance Area: The file for Child #1, 2, and 3 contained agreements that were lacking the children's admission date.

Correction Required: An agreement shall specify the date of the child`s admission.

Noncompliance Area: The file for Child #5 contained a Health Assessment that was more than 12 months old.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: The files for Child #4 and 5 contained Health Assessments that were lacking statements that the children were able to participate in child care and appear to be free from contaigious or communicable disease.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: The file for Child #3 contained a Health Assessment that was obtained more than 60 days after the child's enrollment date.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: The family child care was lacking a plastic lined hands free covered can to place soiled diapers in.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: The file for Staff #1 had a Health Assessment that was more than 24 months old.

Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file for Staff #1 was lacking previous TB test.

Correction Required: A facility person`s record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: The file for Staff#1 was lacking proof of High School Diploma or GED.

Correction Required: The operator shall have a high school diploma or a general educational development certificate and submit proof to the appropriate regional office of the Department at the time of registration renewal.

Noncompliance Area: The file for Staff #1 was lacking a State Police Clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).
