Noncompliance Area: The facility file for the cat named Mumma lacked verification of a current rabies immunization. New rabies immunization was due by 4/15/2019. The facility file for the dog named Harley lacked verification of a current rabies immunization. New rabies immunization was due by 4/11/2019.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: There was more than six months between reviews of the emergency contact information and fee agreement on file for child #1. Reviews were 1-16-2019 and then 1-27-2020.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The file for child #1, who is of preschool age, lacked an updated health assessment which was conducted in the last 12 months. Last health assessment was completed 2/26/2019.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record must contain initial and subsequent health reports.

Noncompliance Area: The most current immunization record on file for child #1 lacked verification of flu shot within the past year as in accordance with the schedule recommended by the ACIP. The most current immunization record on file for child #2 contained verification of 1 Rotavirus when 2 are required, 1 DTAP when 2 are required, 1 HIB when 2 are required, 1 Pneumococcal when 2 are required, 1 Polio when 2 are required, and verification of a flu shot within the past year as in accordance with the schedule recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: On the date of inspection, it was observed that there was not an operable smoke detector in the basement because the smoke detector was not hung on the wall as required by manufacturer instructions and was sitting on the furnace. This condition at the facility poses a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.. Demonstration of Compliance with 34 Pa. Code 403.23 is required. . The operator will verify correction with an agent of the Department after installing an operable smoke detector in the basement.

Noncompliance Area: The last review of the emergency plan was more than one year ago and dated 2/26/2019.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: The file for staff person #1 lacked 3 of the required biennial clock hours of child care training and were due in the file by 1/1/2020.

Correction Required: A staff person shall obtain an biennial minimum of 12 clock hours of child care training.A facility person's record shall include records of training required by the Department.

Noncompliance Area: Observed that the wooden front steps, wooden porch floor, and wooden porch railings had peeling paint. These areas are used by the children in care to access the family child care home.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Observed a child's chair which had foam on the seat and was covered with plastic. Half of the plastic was coming off of the chair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the back yard, observed an embedded swing set which lacked loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Observed an infant pack n play which was not labeled with the child's name.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The facility refrigerator lacked a thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Based on a conversation with the operator and observation, child #1 who is an infant, sleeps in a bouncy chair.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: The facility lacked a conspicuously written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of emergency.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: Child #1, an infant, lacked a written statement from the parent giving the formula and feeding schedule.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.
