Findings: PER ALLEGATION and based on facility documentation and staff interviews, staff members were not consistently logging diaper changes of the enrolled infants. A staff stated in the morning they did not log the diaper changes and were not aware it was required. **This was previously cited on 4/17/2018.**

Findings: During the inspection of the infant room, the Team Leader asked to see where an infant's diaper changes were documented. A Staff stated diaper changes are documented on each child's daily sheet and given to the child's parent at the end of the day. The Staff stated the facility does not keep a copy of each child's daily sheet.

Findings: PER ALLEGATION and based on staff statements to the Team Leader, Staff 3 has been observed slapping enrolled children on their hands as a form a discipline. The Staff stated they observed Staff 3 slap a child's hand. The Staff member stated they told Staff 3 that slapping a child is not acceptable and to never touch a child in that manner.
