Initial Comments: The following deficiencies were observed at the time of the Monitoring Inspection conducted on 1/16/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The Empower Self Assessment was emailed to the provider. The Emergency Disaster Contact Form was completed on site. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that a statement regarding inspection reports is in the Statement of Services. 2). Ensuring the first aid kit in the van has adhesive tape. 3). Ensuring the floor of the van is maintained in a clean condition. 4). Initial Comments: The following deficiencies were observed at the time of the compliance inspection conducted on 1/24/24 and are subject to changes pending programmatic review.
