All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. There was food on the floor and the high chair had not been cleaned. Since this program has been closed for the summer and this child care space is separate from the living space, everything just needs a deep clean. There were a few broken toys on the playground small parts that could be a choking hazard to the children under 3 years of age.

Indoor stairs with more than two steps were not made inaccessible to children two years of age or younger. The loft in the back playroom has a gate on it but it was open and children under the age of two have access to this.

Child had an epi pen with an action plan signed by the parent but not signed by the doctor. Benadryl did not have a permission slip filled out at all.
