Mt. Sylvan Preschool has been open for 50 years and is still recognized as "the best of the best" in early childhood education. We offer breakfast, lunch and a snack daily as well as extra curricular activities such as soccer shots. We participate in community service projects monthly as we believe in taking care of our community. Mt. Sylvan is known as offering a top of the line curriculum where our children are nurtured and given everything needed for their next step in education. We have remained open during COVID-19 and are in compliance with all mandated rules. Call us at (919) 477-7389 and ask for Ms. T or Ms. Lynn to set up a tour. The last documented approved fire inspection was dated 11/4/22. Durham Fire Inspections had not been contacted to request an updated inspection.

Individual responsible for a group of children did not meet the preservice requirements for a lead teacher or teacher. During today's visit the "Soccer Shots" coach, who was not an employee of the center, was responsible for a group of children.

Prior to employment a Criminal Background Check was not completed. There were no Criminal Record Check clearance letters on file for the speech therapist, who was present today, or for the "Soccer Shots" coach. Before the close of the visit, "Soccer Shots" emailed the director Criminal Record Check clearance for the coach present today.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A child in classroom space #B had Diazapam with a medical permission form that expired on 1/4/22. The director stated that the child was no longer taking the medication.

Children were not adequately supervised at all times. The consultants observed that children were not being adequately supervised on four occasions during today's visit.
