The depth of the loose surfacing was not based on critical height of the equipment. Mulch is needed on the playground area. The mulch did not meet the critical height of the equipment.

A safe indoor and outdoor environment was not provided for the children. Five metal stakes that hold the plastic border surrounding the mulch were sticking up and were a tripping hazard. The deck used for children to exit the classroom to the playground has wood that is splintered in 5 different areas as well as chipping paint. The cap is not attached to one vertical fence pole between playground 2 and 3. This is a violation of requirement in 10A NCAC 09 .0601(a). A section of the lattice was disconnected from the deck.
