This includes (1) a detailed medical plan from the parent as to when medication is to be administered (2) authorization from the doctor with speci9fic medication and dosage, and (3) a medication log signed by the parent detailing symptoms for administration of inhaler followed by a section for the daycare to complete showing date, time, dosage, comments and initials.
