Findings: During a review of children's records, no evidence of a two year old boy having had any blood lead tests performed with subsequent results were found. Please provide the results or notify the parents, in writing, of the need for blood lead testing and results. It was also found that a 6 month old girl does not have the correct amount of immunizations for her age. Findings: According to the Provider's written records, she conducted both a fire drill and an emergency disaster drill on the same date and at the same time on 11/19/2025. These are separate drills and should be conducted as such. A fire drill must be conducted and recorded at least once per month.
