Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In space number one (#1) there were twelve children from ages two (2) to four (4). Erykah Cooke was the only teacher in the room. In space number two (#2) a two-year-old child was brought into the classroom to use the bathroom. There were 18 children ages four (4) and five (5) in the room with one teacher.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space number one (#1) there were three (3) electrical outlets that were not covered by safety outlet covers. The teacher immediately covered the outlets.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space number one (#1) there was a can of Lysol Disinfectant aerosol spray in an unlocked cabinet. The teacher moved the spray into a locked cabinet. In space number two (#2) there was a can of Hospex Disinfectant spray that was in an unlocked cabinet. The spray was moved into a locked cabinet.

Corporal punishment was used. [Omit item 0903 for religious sponsored centers operating under GS 110-106 with an exemption on file with the Division.] A child was hit on the hand by Marsha Dunn the teacher in space number two (#2).

On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Ms. Cooke was hired on December 28, 2022, but the TB test on file was dated January 3, 2023, which was after the first day of employment.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Ms. Platt-Marks last updated her emergency information form on September 3, 2019. Ms. Platt Marks completed the emergency information form during the visit and it was placed in her file.

Prior to employment a Criminal Background Check was not completed. Donald Battle was providing transportation in the mornings and in the afternoons. Mr. Battle did not have a Criminal Background Check on file.

New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Ms. Cooke did not have documentation of completing at least 16 hours or orientation within the first 6 weeks of hire.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Ms. Cooke had documentation of First Aid from the National CPR Foundation completed on January 4, 2023; however, the class was taken virtually and was not from an approved organization.

Vehicles used to transport children enrolled in the child care center did not comply with all applicable State and federal laws and regulations. The van used to transport children with the NC 49073-W license plate, did not have a North Carolina registration card on file for review.

First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. There was not a First-Aid kit on the van, however Ms. Carr-Battle was able to put one on the van before I finished the visit.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The children who were being transported did not have their pictures or emergency information on file during transportation.
