The success of our curriculum and our program as a whole can also be attributed to parent participation being highly encouraged. Our curriculum and monthly planning fosters strong parent/teacher relationships through verbal communication, LuvNotes™ (electronic daily messaging), posted classroom schedules and lesson plans, newsletters, conferences, events and programs. Parents can even use our Kiddie Cams™ to continue their involvement by watching an activity or event at its planned time when they cannot be there in person.

The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1. Bird feces on the turf and climbing structure. 2. Staff files must be complete and should include previous child care experience. 3. The menu must reflect the specific items to be served to enrolled children. 4. Medication forms must have specific reasons to administer medications. 5. Defrost the freezers. 6. The weekly menu must be posted. 7. Cleanliness of the diaper sink in the 1s room. 8. The current license must be posted.

Initial Comments: The purpose of the inspection was to conduct a Complaint investigation 90370 on 9.23.2024. A full inspection was not conducted at this time. A Plan of Corrections is not being accepted at this time. Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 8/12/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please submit a fall zone test or specifications of the Astro turf on the Preschool playground. 7 of 7 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Children's immunization records. 2. Defrost the freezer in the Hundred Acre room. 3. Staff signatures on the 10 day new staff training. 4. A detailed transportation statement and pesticide statement. 5. Occupied cribs must be located 2 feet apart. 6. Sippy cups must be labeled with an enrolled child's first and last name. 7. Pacifiers must be labeled with an identifier. 8. Wood on the Toddler playground. 9. Lesson plans must be dated for the current calendar week. 10. Initial Comments: The purpose of the inspection was to conduct complaint #0068095 investigation on 01/22/2024. A full inspection was not conducted at this time. Name of Compliance Officer#1: Jennifer Flicker, and Name of Compliance Officer #2: Fred Geyser. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Ratios observed were: Infants: 2:8, One year olds 2:11 and 2:13, Two year olds 2:13, and Three-four year olds: 2:17. There were 7 staff interviewed during this investigation. Others interviewed: The complainant. Documentation observed were rosters, the Scottsdale Police Incident Report related to the allegations and the Licensee's Performance Improvement Plan for the alleged Staff. One staff file was reviewed and the fingerprint clearance card for the staff was verified to be valid via the DPS website. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that 1 of the allegations was substantiated. The ratio observed was: Infants-2:7, 1 year old room-3:12, 2 year old room- 1:7, and 3- 4 year old room- 1:13. There were 3 staff interviewed during this investigation. Others interviewed: The Compliance Officer left a detailed voice message for the complainant, requesting additional information. The complainant did not return the call. Documentation observed were 2 staff files, the diapering log and rosters. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that one of the allegations was substantiated. The following deficiencies were observed and are subject to changes pending programmatic review.
