Findings: Based on staff statement, a child's statement, and written facility documentation, a staff member placed the health, safety, or welfare of an enrolled child at risk of harm by grabbing a school age child by the back of the child's neck and sitting the child in a chair. After the incident the child went into another classroom. The child was crying and told the teacher in that room that he was grabbed by his hair and his neck and put in a chair for cutting in front of another child during musical chairs. The facility suspended the staff member for 5 days for "causing physical harm to students."
