Staff did not complete an incident report when a child sustained a bump on the head on August 4, 2025.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. In Space 2, one child's twelve month authorization for permission to administer medication had expired on 4/8/25. One child's diaper cream stated "as much as needed" vs. a specific amount in Space 1.
