| Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. On the playground, two tricycles were observed to have cracked plastic seats. At the main gate entrance of the playground where the gate door meets the fencing, two bolts which secure the fencing were protruding inward from the gate, one half inch beyond the nuts, presenting a potential safety hazard. Three rusting bolts were observed on the nonstationary playground structure to the back right of the playground, and one bike/wagon was observed to be rusting. Mildew was observed on nonstationary playground structures. | |||

| All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The handlebars and bell on one tricycle is beginning to rust. A dryer vent cover was detached from (broken off from) the siding of the house that borders one side of the playground. The broken area was sharp to the touch. | |||
