Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 3/6/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The following was discussed but not limited to: 1) Items stored in a bathroom are required to be stored inaccessible to children and in a closed cabinet or on a shelf in a covered, waterproof container. 2) New rules effective 8/2025, 3) All staff are required to submit DCS clearance through the DCS portal that was effective in July 2024, and 4) Room arrangement as related to ensuring designated exits are not obstructed. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 3/7/2025 and are subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 4/10/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but not limited to: 1) Removing the Brown colored calcification deposit stain that is on the base and the side of one of the toilets in the boys' hallway bathroom, and 2) Obtaining the DCS clearance results for staff #1 that were submitted in 5/2019. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.
