There was a diaper cream and prescription cough medicine with expired permissions. Medications and topical creams were removed and discarded during the visit.

A current activity plan was not posted for each group of children for reference. There was not an activity plan posted for school aged children.

A safe indoor and outdoor environment was not provided for the children. Indoors, there was chipping paint and drywall in the licensed space for 1 year olds and 4 and 5 year olds. Carpets in the licensed spaces for 4 and 5 year olds and in the space for school aged children curled up at the ends causing a tripping hazard. Outdoors, there is a deep hole near the drain in the middle of the playground. There are also two gutters that are sharp on the ends.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Three outlets were uncovered in the licensed space for two year olds.

Within the first two weeks of assuming responsibilty for supervising a group of children, staff did not complete at least 6 hours of training on topics outlined in this rule. There was no documentation of orientation that should have been completed by 9/12/22

Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. There was not a working thermometer located in the refrigerator. When I opened the door it did not feel cool inside.
