The center did not have developmentally appropriate equipment and materials accessible daily. In space three the shelves had most of the toys removed and children did not have enough toys to play with.

A safe indoor and outdoor environment was not provided for the children. The small playground area near the toddler classroom had fencing that had sharp edges sticking out of it. The large playground area had paint chipping and it was rusted. There was a hole on the fence line that a small child could get through.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A container of ZEP wipes was found under the cabinet is space space four.

A drug or medicine was administered after its expiration date. In Space three a medication form had expired on February of 2023.

When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. In space three and six there were two epi pens that the documentation was expired.

The depth of the loose surfacing was not based on critical height of the equipment. The large playground equipment had two inches of mulch under it.

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Medical information was stored in a staff members file.
