A safe indoor and outdoor environment was not provided for the children. Inspection of the outdoor playground found the following hazards: pool noodles torn off the top of the fence and zip ties on the playground, broken toddler basketball hoop, thin root attached to the ground at both ends that was loose that was a tripping hazard, chain link fence at gate separated from pole creating a hole that children can get out as well as the chain link untwisted sticking out into the playground that can cause injury, and trash (fast food bags and cups) in the fenced in area.

Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A playground inspection was not documented as completed since November 30th, 2023. This required to be completed every month

Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. A preschool age child transported by Wake County did not have the correct transportation paperwork on file giving them permission to transport the child from the daycare.

All records required were not available for review by a representative of the Division. Facility files (staff, children, program) were not readily available when asked. Staff was unable to obtain the files because leaving their classroom would have left the space out of ratio. Someone from the unlicensed space was contacted during the visit and passed them over to the childcare during the visit.

Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. On arrival on top of the bookshelf between the classrooms I observed two cups of coffee, one Bojangles cup with a straw in it and one can of mountain dew soda that were visible to the enrolled children.

Children were not adequately supervised at all times. On arrival to the childcare facility I observed three teachers in the classroom for children ages three through school-age. As I walked through the door one of the teachers that was holding a child was observed returning to her classroom space where she cares for toddlers ages one and two. No staff person was observed in the toddler room there was only six toddlers in the room.

I have used this provider for more than 6 months

Please don't send your children there, they are not professional , the director thought it was appropriate, to say tome "my daughter was Messy" when discussing with me an incident that happened at the center, but they are mostly family there and of course because my daughter had an issue with her nieces , she chose to be biased and my daughter felt that on regular basis, i have reported them , there is a lot of questions with that place, look at their inspections and violations YOU HAVE BEEN WARNED
