Findings: There seems to be a significant amount of flies outside. To the extent when outside on the play ground you are attacked by them. Specialist is not able to determine the cause of the flies. There is an overwhelming smell of feces in the air but nothing was present in the providers yard.

Findings: Specialist witnessed child care areas that contained lots of dirt, clutter and food pieces from previous meals served. The carpet was dirty, the flooring around the table that the children eat on was littered with food debris. The laundry was spilling out into the hall, a child care area, and a tripping hazard to the children. The refrigerator in the child care area was in need of cleaning. Spilled items were stuck to the bottom door area. The clutter on the kitchen counter is limiting the amount of space available that can be sanitized appropriately and used for food preparation. Findings: Specialist witnessed gate missing from kitchen to living room allowing children access to non licensed areas. The door to the laundry room was open allowing children to non licensed area. The door to the office (across from laundry room) was broken allowing access to the room. This room is a significant hazard to any child. Please see photos. Lock was not latched on hot tub, allowing children access to hot tub that is fully operational.

Findings: The lock on the cabinet under the kitchen sink is again, not working. This is a repeat non compliance. The children have access to cleaning supplies. ie. Lysol spray, Lysol wipes and other cleaning items. Several drawers either did not contain locks or the locks were not functioning properly allowing the children access numerous hazardous items. ie, "Pop its", electric knife blades etc.

Findings: The outdoor playground steps leading to the slide has a step that is in need of repair. It is not sturdy enough to hold the children. The timbers surrounding the playground are were not anchored causing a tripping hazard for the children. The horse shoe pit was not protected causing a tripping hazard for the children. The metal rod was sticking up and not covered. The fire pit contained ash from previous burns and was accessible to the children. A container containing poison to kill ants was located in the back yard. The provider has a large piece of lattice on the back deck that needs to be put in place. The provider has a hot tub with a cover that is to be locked at all times during child care. The cover was not completely latched. The specialist was able to open the cover and see it was filled with water.

Findings: Provider did not have the self closing latch on the gate leading to the pool. The provider did not have the pool alarm working during child care hours.

Findings: Specialist witnessed numerous items that the children had access to. ie knife blades, medication was on the kitchen counter, hand sanitizer, cleaning agents(Lysol spray), pitchfork, ant poison, motor oil, lightbulbs, glade plugin oils.
