2026-05-07 On May 7, 2026, the licensee submitted falsified training certificates to the licensing specialist. Based on investigation and interview, it was determined that at least two certificates, Skills and Strategies for the Child Care Teacher and Fundamentals of Infant and Toddler Care, were altered to appear that the child care worker was qualified as a lead teacher. 2026-05-07 On May 7, 2026, the sole child care worker observed supervising the group of children in the Moon Munchkin room was not qualified as a child care teacher. Upon review of the "sign in sign out sheet" received from the licensee, it was determined that the unqualified worker was alone with in the Moon Munchkin room, with a child or group of children, for approximately 15 days from April 14, 2026, to May 7, 2026. 2026-05-07 Based upon observation, the moon munchkin room failed to follow their written program of daily activities on May 7, 2026 for a four year old in care, instead of matching letters, splat art, art supply patterns, clay names, community artists, pointillism with Seurat, and build the beat art band, the child in care was offered infant age toys and books. Near the end of the visit the child was given one bin of interlocking blocks by the front desk child care worker. Violation
