Noncompliance Area: On 09/21/2020, the file for child #1 contained a health report which was not updated at least once every 12 months for a preschool child. The file contained a current health report dated 08/20/2020 with the previous health report dated 02/18/2019. An updated health report was due prior to COVID suspended regulations.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: On 09/22/2020, observed drawers in the kitchen to the right and left of the oven containing sharp objects such as knives, cork screws, kitchen shears, etc. with broken safety latches which were no longer securing the drawers, allowing access to the items within the drawers.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: On 09/22/2020, observed a space heater in use on the kitchen counter. The operator did not have documentation from the local ordinance that use of a space heater is permitted.

Correction Required: Portable space heaters, if allowed by local ordinance, may be used while children are in care, if the units are used in accordance with the manufacturer's operating instructions.

Noncompliance Area: The file for child #1, a preschooler, lacked an updated health report once every 12 months; the file contains a current health report dated 09/05/2019 with the previous health report dated 01/11/2018.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: Observed a drawer with a broken safety latch containing sharp knives in the kitchen next to the oven which were accessible to children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

The files for children #4 and #5 contain documentation of a current child service report completed on 06/20/2018 with the previous child service report completed on 10/25/2017.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: Observed a tube of antibacterial ointment in the front pocket of a backpack which was located on the floor of the playroom which was accessible to children.

Noncompliance Area: Observed electrical outlets which lacked protective receptacle covers in the corner of the living room which were accessible to older toddlers and preschool age children in care.

Noncompliance Area: Observed a fireman's pole approximately 6 feet high in the outdoor play space accessible through an opening on a platform approximately 4 feet high in the climbing structure which lacked loose-fill or unitary playground protective surface covering beneath the opening and the pole.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Observed sharp knives on the counter and in an unlocked drawer next to the stove which were accessible to children. Observed uncovered bolts on the fence on the left side of the fence in the outdoor play space creating sharp points.
