Noncompliance Area: While reviewing facility files for the renewal the inspector found that household member #1 turned 18 in March and did not obtain the required state police clearance until 11/02/20, Child abuse on 11/6/20, NSOR on 11/17/20 and FBI still in process as of 12/8/20.

Correction Required: At renewal, the legal entity shall submit clearances required by the CPSL for each individual 18 years of age or older who resides in the facility at least 30 days in a calendar year if the individual attained 18 years of age following the date of the previous application for a registration certificate.

Noncompliance Area: While at the facility unannounced on 10/16/19 for the renewal inspection the inspector observed child file #1 and #2 to both be missing the necessary information for health insurance coverage and policy number as is required.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: While at the facility unannounced on 10/16/19 for the renewal inspection the inspector observed the file for staff #1 and found that there was no trainings on file towards the 12 biennial hours necessary.

Correction Required: A staff person shall obtain an biennial minimum of 12 clock hours of child care training.

Noncompliance Area: While at the facility unannounced on 10/16/19 for the renewal inspection the inspector observed a missing outlet cover in the room across from the downstairs bathroom.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: While at the facility unannounced on 10/16/19 for the renewal inspection the inspector observed that the kitchen stove was accessible to the children in care.

Correction Required: Hot water pipes and other sources of heat exceeding 110 F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Noncompliance Area: While at the facility for the renewal the inspector reviewed the file for child #1 and noted an enrollment date of 6/7/18 and the health assessment was dated 10/19/18, which is over 60 days following the first day of care at the facility.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: While at the facility for the renewal the inspector reviewed the file for staff #1 and noted that the last health assessment on file was dated 8/30/16, which is over 24 months old.

Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: During the inspection on 10/25/18 the inspector observed Aqua Net hairspray and Hydro peptide lotion out on the counter in the bathroom, and within reach of children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: Child #1 did not have an agreement on file at the facility as is required.

Correction Required: An agreement shall be signed by the operator and the parent.A parent shall receive the original agreement. The facility shall retain a copy of the agreement.

Noncompliance Area: Child # 2 did not have an emergency contact form on file at the facility as is required.

Correction Required: Emergency contact information is required for each enrolled child. Emergency contact information must reference who shall be contacted in an emergency.

Noncompliance Area: Child file #1 was missing health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits,

Noncompliance Area: Child file #1 was missing the necessary signatures for emergency medical care and first aid treatment on the emergency contact form.

Correction Required: A child`s record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.A child`s record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Noncompliance Area: Child file #1 was missing the necessary signature for transportation as is required on the emergency contact form.

Correction Required: A child`s record must contain signed parental consent for transportation, walking excursions, swimming and wading.

Noncompliance Area: Staff #1 indicated that she did not have a current first aid certificate on file at the facility as is required.

Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable.
