Noncompliance Area: Observed six rest mats and two pack n plays which lacked a label with the child's name and the rest equipment was not being used only by the specified child. Staff #1 stated that she was cleaning each mat between each use and the children were not using a specific rest mat.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: There was more than six months between the previous and most current and updated health assessment on file for child #2, who is a young toddler.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddlerA child's record must contain initial and subsequent health reports.

Noncompliance Area: The file for child #2 lacks verification of a flu shot within the past year.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: There was no written statement from the parent giving the formula and feeding schedule for child #1, who is an infant.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: A copy of the application regulations under which the facility is certified was not posted in a conspicuous location used by parents.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: The file for staff #1 lacked verification that first-aid training was renewed on or before expiration date of 2/17/2020 and was due prior to 3/6/2020 (regulatory suspensions.).

Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable.A facility person's record shall include records of training required by the Department.

Noncompliance Area: First aid training expired 2/17/2020 for staff #1, who is the only family child care staff person and was due prior to 3/6/2020 (regulatory suspensions.) Therefore, verification is not present in the file that one or more persons competent in first-aid techniques are present at the facility when day care children are in care.

Correction Required: One or more persons competent in first-aid techniques shall be at the facility when day care children are in care.

Noncompliance Area: The emergency contact information on file for child #3 lacks the health insurance name and policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The file for children #1 and #4 lack verification of a flu shot within the past year. Based on the age of the child, the file for child #2 contains verification of one Heb B shot when the child is recommended to have two. Based on the age of the child, the file for child #3 contains verification of one Rotavirus shot when the child is recommended to have two, one DTAP shot when the child is recommended to have two, one HIB when the child is recommended to have two, one PCV 13 when the child is recommended to have two, and one IPV shot when the child is recommended to have two.

Correction Required: The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: The files for household persons #1 and #2, who are now 18 years of age, lack a Child Abuse Clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: On 07-19-2018, at approximately 10:00 AM., observed staff #1 providing care to 3 preschoolers and 1 young school age child in the outside play space. Provider's young school age child was also present. Approximately 2 minutes later, observed staff #1's 17 year old come outside and bring out child #1, an infant, to the provider. Based on a statement from operator, she was changing the infant's diaper when her young school age child opened the door and the other children ran outside. Staff ran to get them and told her 17 year old to finish changing the diaper and then bring the infant outside.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3290.51-3290.52

Noncompliance Area: The facility lacked a conspicuously displayed written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: The file for child #1 lacked a fee agreement.

Correction Required: A child's record must contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement.
