Surfacing did not extend six (6) feet beyond the external limits of the equipment or 3 feet for equipment used only by children less than 2 years of age. The ground cover is mulch, The mulch has weeds growing in it and that makes the mulch very compacted.

All equipment and furnishings were not in good repair. The climbing equipment has many places of peeling paint, the steps up the climber is broken and sharp. There is a car shaped piece of equipment that has broken slats. Some of the slate were replaced, but not all. This is an entrapment danger.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence has large holes in it and is coming down in places. These areas are entrapments and has protrusions.

The depth of the loose surfacing was not based on critical height of the equipment. The mulch around all of the climbing structures does not have the required depth of surfacing. It is not the required depth, nor is it the required 6 feet from the critical height of the equipment. Grass/weeds have crept into the mulch decreasing the required fall zone.
