In the outdoor learning environment, there was a pink car that children could ride on, the seat was broken off and the battery and wires were exposed and in the sandbox the plastic lining of the sandbox was exposed and loose, this could potentially be a suffocation hazard.

Ms. Britt did not have documentation for all the required health and safety trainings. Ms. Britt only had the following on file: Prevention and Control of Infectious Diseases completed on February 24, 2022, and Prevention and Response to Emergencies Due to Food and Allergic Reactions completed on February 25, 2022.

A safe indoor and outdoor environment was not provided for the children. In the outdoor learning environment, there were two active anthills along the pathway. You sprayed the anthills immediately. There was also a broken white mesh chair. You removed the chair from the playground.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was an electrical outlet in space number four (#4) not cover by a protective cover. You immediately covered the outlet with a protective cover.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Ms. Britt, Ms. Taylor, and Ms. Williams did not have documentation of completing CPR training.

All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Ms. Britt, Ms. Taylor, and Ms. Williams did not have documentation of completing CPR training.
