Finds: CITED - Moderate
R381-100-6(12)(a)-(b) - Children's admission and health assessment reviewed, updated, signed, and on-site The provider was out of compliance with this rule by not having complete children’s records. In the facility, the admission and health assessment forms for two children had not been reviewed, updated, and signed annually by a parent or guardian. This was a repeat rule noncompliance.
