Violation: The facility's outdoor space was not safe for children's activities as evidenced by rusted paint cans, with some cans full of paint, a door with screws protruding out, a piece of a door trim with nails sticking through, a container of joint compound, a wooden child's desk with desk chair that was warped and chipped, a mop bucket filled with frozen water, a broken metal ball hoop filled with various materials which included: pest spray, foam compound, foam installation spray, roofing materials, and pieces of fence. A large tree branch that measured approximately 4.5 feet by 1.5 feet with sharp wooden edges, multiple tree limbs that pose a puncture hazard to a child, and multiple pieces of trash. The hand rail on the staircase to the play space was observed with chipping/flaking paint. An excessive amount of gum balls were present throughout the play area creating a tripping hazard .

The Tuberculosis (TB) Risk Assessment form, revised March 2014, is incorporated by reference in this rule, as published by the Missouri Department of Health and Senior Services, PO Box 570, Jefferson City, MO 65102 and available by the Missouri Department of Health and Senior Services at https://health.mo.gov/living/healthcondiseases/communicable/tuberculosis/tbmanual/pdf/RiskAssessmentform.pdf. If the person has signs or symptoms of tuberculosis, or risk factors for tuberculosis, then testing for tuberculosis shall occur.
