| Findings: A follow-up visit was conducted on 5/7/2021. LS observed outdoor play equipment was removed and there is more space in the outdoor play area. Provider had movers come and haul away some of the older toys. A complaint investigation was completed by LS Nicole Price on this date. Complaint alleges that home has a horrible odor and is dark inside. Complaint further alleges that the children have access to the stove and kitchen, children sit directly in front of the television, a man walked pass the children in a towel and no pants and the daycare is cluttered. During this inspection, it was noted as well that the outdoor play area needs more space and LS has advised that provider remove some of the items and LS advised that provider remove one large slide be removed since it has a nail sticking from it and is not that sturdy. Provider stated that someone was coming to remove the items. LS will conduct follow-up visit. | |||
