Noncompliance Area: An unlidded garbage can in the School Age Classroom was observed to contain food, cups and plates from an earlier meal in the classroom.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: The daily schedule and the emergency transportation plan were not posted in the Preschool Annex Classroom.

Correction Required: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: Training chairs were observed in the hallway restroom and are being used for Toddler Age Children. The training chairs are reported to be emptied and rinsed with water following use and then sanitized at the end of the day. The training chairs are not being sanitized after each use.

Correction Required: A training chair is not a flushing toilet. A training chair shall be emptied and sanitized after each use. An acceptable sanitizing solution is 1/4 cup of bleach combined with 1 gallon of water. A sanitizing solution shall be treated as a toxic.

Noncompliance Area: Hand washing signs were missing from one of the stalls in the boys restroom.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

When asked how many children were present Staff Person #1 indicated 16 or 17. Staff Person #2 reported that they had just come into the classroom. A total of 19 preschool children were counted as being present in the classroom.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

The windows were covered with cloth and this did not allow for the Staff Person to properly see them, which impacts their level of supervision. At approxmiately 2:00pm during afternoon naptime, Children in the toddler room were observed to be resting on mats. The windows were covered with clothe and this did not allow for the Staff Person to properly see them, which impacts their level of supervision. Sources of artificial light were not in place.

Correction Required: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light. The children were observed to be left in the high chairs and when questioned Staff reported that they will keep the children in the high chairs while the older children complete their lesson in another part of the room. This may be up to 45 minutes.

Correction Required: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

Noncompliance Area: Infants have been allowed to sleep in swings for periods of time. This sleeping position is not in compliance with the American Academy of Pediatrics.

Noncompliance Area: The file for Staff Person #2 lacked an initial health assessment and tuberculosis screening prior to starting in a child care taking position.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: The files for Staff Person's #1 and #2 lacked verification that they had received emergency plan training at the time of initial hire.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: The file for Staff Person #2 lacked a state police clearance prior to starting in a child care taking position. The files for Staff Person's #1 and #2 lacked completion of a Mandated Reporter Training.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: One electrical outlet in the Toddler Nap Room and three electrical outlets in the hallway were observed to be missing protective receptacle covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: A garbage can containing food items was observed to be missing a lid in the Preschool classroom.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Noncompliance Area: Hand washing signs were missing from the potty chairs in the boys and girls restroom off the hallway.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
