The diaper table was cleaned with soapy solution, it was sprayed with disinfecting solution however that solution was immediately wiped off rather than allowed to air dry for two (2) minutes as required.

A current activity plan was not posted for each group of children for reference. The posted weekly activity plans did not have dates for the current week. For plans that were posted they were not being fully followed, as some of the books/activities listed were not available in the classrooms.

The activity plan was not designed to stimulate emotional and social, health and physical, approaches to play and learning, language development and communication, and cognitive development for each group of children in care. The program has activity plans from a purchased curriculum however not all areas of development are addressed on those plans. Supplemental plans were not posted or completed. NC Foundations was not being used as part of the weekly planning.

Caregiver did not make provision for constructive guidance and setting limits that the child can understand and that foster the child's ability to be self-disciplined. In room 3 the space is set up with a table in the middle of the room and materials along the wall. Almost constant running by the children was observed.

Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. Staff assisted children with wiping noses however hands were not washed each time this occurred.

Toys and other mouth-contact surfaces were not cleaned and sanitized daily when used or cleaned more frequently if visibly dirty. Rm 2 did not have a bin labeled for dirty/mouthed toys. When asked the teacher stated a basket on the shelf was used for toys that needed to be sanitized. A child was observed chewing on a foam puzzle piece.

A safe indoor and outdoor environment was not provided for the children. Squeeze paint was on the art shelf in the room with 2 and 3 yr old children. The squeeze paint was labeled not for use by children under 3 yrs old. It was removed from the art shelf during the visit. A blind in room 3 had a broken slat which should be raised to be inaccessible to children.
